چکیده

almost incredulous envy in countries where such addiction is still a serious problem. These measures have been wholly salutary, but future legislation concerning the safety of drugs is bound to be more difficult, because no drug is ever entirely safe and its safety must be related to the purpose for which it is to be used. For example, a high degree of toxicity might be tolerated if a drug cured or stayed the progress of an otherwise fatal disease, but no significant toxicity would be permissible in a drug used for a trivial condition or if it was shown to be worthless. This brings up the whole question of efficacy and of relative efficacy; and who is going to dogmatize on this ? Again, who is going to say that the occasional fatal toxic reactions which may result, for instance, from the use of psychotrophic drugs in depressive illnesses are or are not greater than the danger of an increased incidence of suicide if such drugs are forbidden? Doubtless a committee of experts will advise the appropriate Ministers, and if experts are occasionally wrong they are less often wrong than non-experts. Nevertheless, we interfere with the prescribing doctor's final freedom of decision at our peril in a free democracy. It is easy to set up a sort of pontifical therapeutic Establishment ; but Establishments-Aristotle and Galen, for instance-have not always been in the van of progress.

ABSTRACT: A consecutive Series of 522 patients (mean age 40.36 years ,female 70.9% ,male 29.1% ) with rheumatic valvular heart disease were studied during a period of seven years ,with particular reference to understanding the prevalence and risk factors of atrial fibrillation (AF) and systemic arterial thromboembolism (SAT). AF was observerd in 165 (31.6%) patients .The frequency of AF were ...

Right sided heart evaluation after successful mitral valve replacement.

Introduction: It is well-documented that right-sided heart dysfunction and significant tricuspid valve regurgitation (TVR) have adverse effects on patient outcomes after left-sided heart valve surgery. Therefore, the evaluation of right ventriclular (RV) function and TR severity in patients who had undergone mitral valve replacement (MVR), associated with/without concomitant su...

High-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient

Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...

Pulmonary artery aneurysm associated with mitral valve disease: a grave prognostic sign.

Seven patients with advanced mitral valve disease and systemic pulmonary hypertension subjected to mitral valve replacement without survival are presented here with the common denominator or the characteristic additional finding of aneurysmal dilatation of the pulmonary artery. During the same period 16 other patients with similar degree of valvular dysfunction and systemic pulmonary hypertensi...

Role of Transthoracic Echocardiography in the Evaluation of Patients with Retinal Vein Occlusion

Introduction: Retinal vein occlusion  is a common vascular disorder disrupting vision. Two basic types of RVO are branch retinal vein occlusion and central retinal vein occlusion (CRVO).  Retinal vein occlusion is a multifactor process including systemic illness and local retinal factors.RVO may be associated with atherosclerotic risk factors. We analyzed the role of 2 dimensional transthor...

Early Results of Mitral Valve Replacement in Severe Pulmonary Artery Hypertension—An Institutional Prospective Study

Introduction: In patients undergoing surgery for mitral valve replacement (MVR) for valvular heart disease, pulmonary artery hypertension (PAH) has been considered a major risk factor. In this prospective study, we have studied the early hemodynamic changes and post-operative outcomes of MVR among patients with severe PAH. Methods: 68 consecutive patients who underwent mitral valve replacement ...
